A 51-year-old man has died while carrying out routine maintenance work at a warehouse.
Peter Hudson, from Coventry, was working at RS Components, which is based at Bermuda Business Park, Nuneaton, Warwickshire.
Mr Hudson, who was an engineer with the company, was working on automated storage equipment when the accident happened on Sunday.
The police and Health and Safety Executive (HSE) are investigating.
A spokesman for the company told BBC News that Mr Hudson, who had been with the company for more than two years, would be "greatly missed".
"RS Components are fully co-operating with police and Health and Safety Executive to assist them with their investigations," said the spokesman.
"Our thoughts and sympathies are with Peter's wife and family and we shall do everything to provide practical support."
