DIY giant B&Q has been fined £550,000 and ordered to pay £250,000 costs after a woman was crushed by a forklift truck while shopping.
Pamela Jean Hinchliffe, 69, from Poole in Dorset, died after being crushed at the firm's Fleetsbridge store in 2001.
Earlier this year, B&Q was found guilty of five counts of breaching health and safety legislation and failing to ensure safe systems of work.
The company was cleared of three counts relating to the safety of employees.
Mrs Hinchliffe died from internal injuries after a driver reversed his forklift into her, leaving her pinned against a shelf unit.
